In brief: Tonsil discomfort
You certainly had a complete workup.
Tonsillith or tonsil stone is a possibility. Do you smoke? You may want to consider tonsillectomy for definitive diagnosis and potential treatment.

In brief: Tonsil pain
Agree w dr. Azar. Only thing missing (as suggested) is possible biopsy.
Also, make sure the x-rays are evaluated thoroughly for abnormalities in the deeper tissues, blood vessels, spine, muscles, and other bony processes at the base of the skull and neck.
