Can ovarian cysts cause persistent upper back ache and bone weakness / discomfort? I also have high ESR and very low ferritin, all other blood normal.

In brief: More than one issue
Simple ovarian cysts should not cause backache.
Low ferritin represents iron deficiency and you may have low blood count. High ESR is usually associated with inflammation, infectious or non-infectious. You have enough symptoms to warrant a consultation with a physician.
