Genetic abnormality. In rare cases, boys with undescended testes can have Klinefelter syndrome. Normally girls are genetically "46XX" and boys are "46XY". Boys with Klinefelter syndrome have "47XXY" or other more rare variants- they are still male, but the presence of the extra "X" chromosome may affect their response to testosterone thereby affecting sexual development. They can also have other disabilities.
