In brief: Immediate physicial

exam. High urine protein can indicate kidney problems or hyperporteinemia.
You will need a repeat urine analysis and a 24 hour urine collection for protein analysis, Please consult a nephrologist.

And it should be obvious that smoking 2 packs a day alone will be enough to prevent you from getting insurance. Talk to your doctor about quitting smoking and make sure you follow through. That is the single most important thing you can do to improve your health and only you can do it, but your doctor can help.

In brief: See physician

Clearly, you need to see your physician for a complete evaluation.
High protein in urine suggests significant kidney disease. The cough may reflect emphysema, chronic bronchitis; even a lung tumor. Meanwhile, stop smoking NOW.
