For medullary thyroid carcinoma: inheritence is the most important factor. For papillary and follicular tumors, radiation exposure is more frequently a problem, although inherited factors also play a role. Having Hashimoto's thyroiditis increases risk of lymphoma by 80 fold!

Radiation to the neck for benign disease- which was common 50+ years ago, rare nowadays.
Sometimes runs in families or part of rare familial syndromes (men) associated with nuclear disasters (chernobyl).. Most patients have no known risk factors.
