Why does my back hurt so badly when I need to use the bathroom? All good on colonoscopy. Have had a couple of abdominal surg. Could it be scar tissue?

In brief: Increased pressure.

You may be experiencing increased back pain if you strain at your stools.
If you do, try stool softeners if necessary, and eat 9-11 servings of vegetables and fruits daily. The latter strategy decreases inflammation and may also help detoxify your body. See your doctor for further evaluation if you're not feeling better. Scar tissue from abdominal surgery is a dubious cause of your symptoms.
