In brief: Very rarely

Only in very rare instances can fibroids be cancerous.
If there is very rapid growth in the size, these are the ones that could be a leiomyosarcomas but again very rare.

In 51 years of practice and thousands of surgeries performed I seen only 1 leiomyosarcoma.

Rapid growth is no longer considered useful in separating benign fibroids from leiomyosarcomas. While no imaging can distinguish, MRI of the pelvis is the best imaging tool we have and fortunately as Dr Brasovan stated leiomyosarcomas are extremely rare. If I see a fibroid that has very atypical features on an MRI, statistically this still should be a fibroid since an atypical presentation of a common condition like fibroids is way more common than a typical appearance of a rare cancer but under these circumstances we would still recommend surgical removal for everyone's peace of mind (and almost always they are fibroids).
