In brief: Skin biopsy

Many pigmentary disorders are diagmosed clinically, by the way they look, and associated symptoms.
If there is doubt, a skin biopsy can be helpful.

In brief: Depends on...

There are many skin pigmentation disorders.
It could be related to systemic disorders such as addison's disease, hemochromatosis, or cirrhosis, etc... If suspected, there are blood test that your doctor can check to diagnose your condition. Many drugs can cause dyspigmentation, such as minocycline, amiodarone, etc.... These are diagnosed based on the clinical pattern of dyspigmentation.
