In brief: Probbly Psychologist

I would find a psychologist or family therapist & ask for a brief phone consult to briefly discuss the issues, enough to determine who should come to the first session.

Many PCPs prefer not to be confronted with family issues so that they can concentrate on medical rather than psychosocial problems. A good clinical psychologist is probably your best bet.

In brief: What type of issue?

If it's a physical issue, a general practitioner. If it's an emotional or psychological issue, go to a psychologist.
