To get scarlet fever you need a strep A infection that has had time to generate the red producing toxin that causes the rash.
I've seen it once (in 3+ decades)in a kid who had an infected cut. I suppose it could also occur if a person has a perianal strep A infection, which is uncommon but does occur.
