In brief: Depends on severity
The vast majority of patients with mitral regurge can be treated medically.
However, when the leak is severe, a more agressive approach is often needed. The mitral valve in this case can be either repaired or replaced. This is done to treat or prevent heart failure. There are several studies that indicated that repair is preferable to replacement.

In brief: Depends
Mitral valve insuffciency, or mitral regurgitation (mr) can range from trivial to severe.
Trivial mr may need nothing more than surveillence. Moderate mr may respond well to medical therapy. Moderately severe to severe mr may require a surgical valve repair or replacement. Studies are currently underway to look at percutaneous methods to repair the mitral valve as well.
