In brief: Grade.
Chondromalacia is non-arthritic damage to the articular cartilage in a joint.
Grade 0 is normal, 1 is slight softening, 2 is small fissures and cracks, 3 is deep nearly full thickness fissures and cracks, and 4 is exposed bone. When you have exposed bone rubbing on exposed bone we call it arthritis. Chondroplasty is surgery to smooth off the rough areas of cartilage to make is smooth.
