In brief: Specialist.

See a non-surgical sports medicine specialist, who can make a diagnosis, and then treat you appropriately.

In brief: Keep moving it

Old injuries of shoulder may leave chronic arthritic changes, chronic bursitis, chronic meniscal tears, rotator cuff changes.
They all could end up being painful and restricting movement. If the shoulder doesn't exercise and move as close to normal as possible, it becomes a 'frozen shoulder'. It will never move right again. Then surgery is an option.

In brief: See a shoulder doc

At your age, shoulder injuries can be fixed very successfully.
Don't neglect it further, as it can have detrimental consequences in the future such as arthritis. See a shoulder specialist to have it worked up and to discuss treatment option.
