Infection is one cause of inflammation, but the bowel can be inflamed without infection. Stool WBC's and associated inflammation can come from anywhere in the digestive system. A colon source is most likely, so colonoscopy makes sense. Bilirubin indicates liver dysfunction/blockage or broken RBC's.
