In brief: Just stop it.
Some people believe that tapering the dosage is a must but it is not mandatory.
If a patient develops contraindications to hormone replacement therapy, we have to stop it. Tapering can be done in a variety of different ways (i.e. Taking it every other day for a week, then perhaps 3x/wk and then stop) but again, it is not necessary.
