In brief: Primary care

It may be optimal to start with your primary care provider to first address the basis of concern that you might have a brain tumor.
The pcp could then determine to whom the referral should be made.

I would review your symptoms with a primary care doctor. They usually will determine if a specialist is indicated.

I would start with a primary care doctor to assess what your symptoms sound like. That doctor can then certainly order imaging if necessary.
