The inventory of depressive symptomatology - self report (ids-sr) is a very good tool for assessing depression severity.
The assessment is open to the public: http://www.Ids-qids.Org/translations/english/ids-sr%20english.Pdf and the scores are interpreted as follows: http://www.Ids-qids.Org/index2.Html#interpretation . I would suggest seeing a psychiatrist for true interpretation, however.

There are certain paper and pencil inventory tests which correlate directly with the signs and symptoms for major depression as in our diagnostic manual.
One such test i like is called the: mongomery-asberg depression rating scale. For me, a score of 30 or higher is a definite sign the person has major depression. I will add that some physical ailments can cause depression, it's best to see a dr.
