In brief: Quality of life
Complications ( brain abscess, vision loss) are possible with untreated sinusitis.
Most cases of untreated sinusitis just lead to chronic sinusitis which can reduce a persons quality of life... Facial pain, pressure, discolored nasal discharge, post- nasal drainage... Etc.

if sinus infection not treated appropriately - the bones that form the sinus cavities can get infected and bone infections take a LONG TIME to heal - or worse - can erode the bone. Also, repeated antibiotics that do not work or are not taken appropriately can increase the chance of a FUNGAL infection - which normal antibiotics ( which only work on bacteria -not fungi-) will not treat...

It is very rare for chronic sinusitis to erode bone.
