I have a friend in the hospital with over 130 mmhg CO2 for the most part is incohearent but is concious. Would a blood transfusion make a difference?

Hypercapnia. Treatment for hypercapnia depends upon its severity, and starts with addressing the underlying cause. If this is ineffective, additional treatment may include: non-invasive positive pressure ventilation (CPAP, BiPAP), intubation and/or mechanical ventilation.
