In brief: It's a difficult one

It's not your fault.
You probably did whatever it takes to have a term pregnancy, but combination of circumstances led probably of having it prematurely. You still can be a good mother, and now it's more valuable for your baby. He/she needs you now more than ever. You have to move forward, and you can not change anything now. What's done is done. Think positively, move on.

In brief: Talk therapy

Guilty feelings after having a premature baby are very common and normal.
Emotional changes after delivery are very common; baby blues are extraordinarily common, and postpartum depression is more likely with prematurity. You should seek a mental health professional for talk therapy; in some situations, medication may be warranted.

In brief: Shame/guilt

Shame is caused by many things and is experienced very differently by different people.
Sometimes people have done things they probably should feel shame about, while many others have not; they only think they should feel ashamed because an authority figure has told them to. Everyone feels shame; it's the rule and not the exception. Find a reputable psychotherapist to speak with in a safe place
