In brief: Stronger cortisone.

Sometimes over-the-counter cortisone is not strong enough to calm down the reaction.
Contact dermatitis would not spread: it just keeps on showing up on the areas being contacted with the offender(s).

In brief: Is diagnosis correct

Contact dermatitis doesn't typically spread without continued exposure.
Spreading rash could be from something ingested or from an infection. If topical steroid cream doesn't work then oral medication might be required.
