In brief: If MR torn ligaments

& x-rays show some spread of the tib/fib & widening of the ankle mortise, i would say surgery.
Doing it now has a much better outcome than if down the road U need a reconstruction. B sure U C a fellowship trained orthopedic surgeon,who practices foot & ankle.

A severe sprain is worse than a fx as you cannot see it and also its progress (by XRays) also is difficult to assess. I would treat it surgically sooner than later, as the results are so much better. These sprains are generally missed or treated incompletely as they are not visible on the XRs. Majority of the sprains are the more commoner (Type 1 & 2) rather than the rarer type 3 or 4.
