In brief: Multifactorial
Pancreatic cancer (adenocarcinoma) is caused by the accumulation of mutations in the dna of cells that line the pancreatic duct.
Smokers, diabetics, and people with chronic pancreatitis are at increased risk, as are some people with familial (genetic) syndromes. Another type, neuroendocrine (like steve jobs) arise from hormone-producing cells and some are related to inherited cancer syndromes.

In brief: Don't really know
We really don't know the cause.
Depending on the location of pancrease (pancrelipase) involved symptoms can be different. Usually silent till it is discovered unless the tumor is at the head of pancrease (pancrelipase) where patient presents looking yellow with change in color of urine and stool. Pain usually in late stages. Best chance of cure if tumor can be cut out. New onset of diabetes may be a symptom ( not specific).
