In brief: Longer, usually

In general, more ingrained troubles take longer (and possibly more intensive) treatment.
Cbt/schema therapy, interpersonal, and psychodynamic/analytic approaches are best known in this area. The difficulty is more a part of the person's general functioning (their 'personality') than, say, a phobia, and the therapist must gradually 'show' the patient without rupturing the working relationship.
