In brief: By symptoms

Usually people have numbness and tingling in their thumb, index and middle fingers with carpal tunnel syndrome.
Some people have swelling and pain as well. There is a test called a nerve conduction study with EMG that can determine if you have carpal tunnel syndrome.

Numbness and tingling at night in the thumb, index, and middle fingers is a hallmark of carpal tunnel syndrome. Usually improves with wrist splints.
