In brief: Increased risk

With barrett's esophagus, you have an increased risk of developing esophageal cancer.
Therefor an individual should be in a endoscipic surveillance program. If the barrett's progresses to high grade dysplasia, then the risk of cancer is so high, then the high grade dysplasia needs to be treated either with endoscopic techniques or esophagectomy.

In brief: It depends

Only a small percent of patients with barrets esophagus develop esophageal cancer.
However, barrets does need to be surveyed every so often by endoscopy to check for precancerous cells.

In brief: Usually not

Ten percent of all autopsy samples show barrets esophagus.
On the other hand barrets does increase one's chances of developing esophageal cancer. Barrets can be treated with rf ablation and should be monitored.

In brief: Not necessarily

But you are at increased risk for it, so frequent surveillance with endoscopes is required.
Don't ignore it!
