Urine should be tested for infection or protein, some children can have blood in urine from infection, others from trauma especially if they have a congenital abnormality, kidney or bladder stone & occasional tumor of kidney or bladder. Red urine can be entirely benign after consuming beetroot +/or some foods or beverages with red dye.
Urate salts in diaper can be red or pink and are benign (safe).
