Not good combination. Think of too much sugar in your blood like maple syrup. Your blood wouldn't flow through veins and arteries as well as it should. The sugar in your blood can cause smaller blood vessels to get clogged and reduce oxygen and nutrient supply to affected areas especially small nerve endings in hands and feet. Poor circulation would only compound this problem. Tissue without oxygen will die off.
No. Diabetics are at increased risk of peripheral arterial disease but this nerve pain could also be related to peripheral neuropathy. At your age and if you don't smoke, doubt it's pad. However if for someone older and who smokes they may need an abi or cta of the lower ext w contrast to determine if there are any blockages.
