In brief: OVERBITE

Overbite usually develop if the child continues to suck his bottle for a long time even at school age.
We prevent this to happen by changing the bottle to cup as early as possible.

In brief: 2 millimeters

Upper teeth should overlap lower teeth vertically (overbite) and horizontally (over jet) by about 2mm. Anything more than this will require professional help.
If overbite/overjet is skeletal, not just dental, will require professional help. Help child discontinue pernicious habits (particularly digit habits) before permanent teeth erupt. Other than that, you're at mother nature's mercy.

In brief: Depends

Likely you mean overjet where the top teeth stick out and leave space.
Overbite is actually how much your top teeth vertically overlap the bottom. Both can be completely genetic due to skeletal form but overjet can be induced by environmental things such as digit sucking or pacifiers. They need to be done with those before 5 and if not appliances can be used to help. See your dentist for help.
