In brief: Insomnia/apnea
You need to be evaluated.
Could be as simple as insomnia or you may have sleep apnea. Chronic medical conditions like hypo/hyperthyroidism, heart disease, and fibromyalgia (to name a few) could also cause un-refreshing sleep...

In brief: Possibly
Unrefreshing sleep is usually a sign that something is interrupting the normal sleep cycles.
Of course there could be a myriad of things like stress, normal aging processes, illness, or a genuine sleep disorder causing it. Also, obstructive sleep apnea could cause it, especially if you are a restless sleeper, snore a lot, or wake up frequently. Ask your doc about a sleep study to look for apnea.
