How to know if I have an anal fissure and I need to change my diet or does this sound like a hemorrhoid that would need surgery?

Pain and bleeding. The fissure is a slice in the mucous membrane of your rectum and is very painful. It beging from straining on hard stools and results in a fissure or tear in the membrane. There may be a stripe of blood on your stools or even droplets in water. Sometines they heal but open on the next strained movement. Surgery involved cutting out the fissure and letting it heal. Recovery isn't bad.
