In brief: Blood sugar

Uncontrolled or poorly controlled diabetes can lead to certain congenital birth defects.
There is a higher incidence of congenital heart defects found in poorly controlled diabetic. A fetal echo should be done on anyone with preconception diabetes or gestational diabetes diagnosed before 20 weeks gestation.

In brief: No data

If and when the sperm swim on and attach to the egg, your blood sugar at that moment has little if any effect on the outcome.
Over the next 2 months, there are significant structural events occurring with the embryo and the most common structural defects associated with poor sugar control would be triggered.Good control can reduce but not eliminate the risk.

In brief: No particular number

Your overall blood sugar control is more of a factor and can be best determined by your hba1c or similar method.
Any female wishing to become pregnant who has diabetes really needs to discuss the risks of diabetes to the pregnancy, mom, and baby before conception if possible. Being in the best control prior to conception and during pregnancy will help ensure a healthy outcome for baby and mom.
