In brief: Heart damage

Central obesity (more fat around the waist) predisposes to what is called obesity-induced cardiomyopathy.
This causes the heart to become weak and stiffen. Obesity may also result in sleep apnea. The low oxygen concentration may in turn cause heart weakness and stiffening, which lead to fluid accumulation in the lungs (heart failure).
