I'm not an expert in treating these disorders, but gamma knife can be an option for avms under a certain size with specific characteristics in eloquent areas of the brain where surgical treatment is not an option.
One can also make the argument for small surgical lesions in non eloquent areas but gamma knife treatment window is longer 2 years sometimes and you go with 2 years of rupture risk.
