They should be looking for any nipple /areola changes such as scabbing dry skin or inversion, especially if new.
They should feel for any new or changes such as masses or lumps. Exams should be performed regularly in conjunction with mammograms or MRI particularly in those with a family history or genetic tendency (braca).
