In brief: Go to the ER!
If someone is having an asthma attack that doesn't respond to their rescue medication, they need to go to the er immediately for treatment.

In brief: Call 911...
If someone is suffering from a severe asthma exacerbation that is not responding to usual treatment, they have status asthmaticus.
This type of exacerbation has a higher morbidity and mortality rate. Such a patient requires immediate medical treatment so 911 should be called and the patient taken to the er stat!
