In brief: Diabetes

Having protein, sugar and blood in urine indicate kidney disease.
That might need biopsy. Also diabetes causes sugar, protein and ketones in urine.

In brief: Worrying

Blood in the urine may require workup for tumor. This is the most worrisome.
I'm assuming that "no medications" means you are not taking unregulated "holistic" medicines (may be liver / kidney poisons). Your physician will make the call as to how to work up the rest based on how far off the values are.
