In brief: Depends

Since your injury is 4 months old the need for surgery depends on how much your wrist bothers you.
If your pain is infrequent, you may want to wait, but if you have pain every day, it is probably not going to get better without surgery. Surgery can be very helpful, but it often takes 3-4 months to fully recover.

Surgery is arthroscopic with debridement or repair and is warranted if you are in pain and have findings of tear on MRI. Debridement would have a recovery time much less than 3-4 months repair would take approximately 4 months due to prolonged immobilization and healing time.
