In brief: Friends
True friends can likely tell when something is wrong, but only a professional can competently diagnose.
A full psychiatric referral is indicated, along with possible medication, and psychotherapy, appropriate to the diagnosis.

In brief: Friends, be friends!
Only a trained professional who is not family/friend can diagnose.
Observations from well meaning, caring & knowledgeable family/friend can be helpful to the diagnostic process but outside of a professional exam, diagnosis and labels are best avoided. Any professional can do diagnosis, but not anybody can be friend! so friends, be friends! you're needed & irreplaceable as such!
