Tophi are collections of uric acid. Medications that lower uric acid shrink tophi if the level of uric acid in blood can be lowered below 6mg/dl and the lower, the faster (but slowly; millimeters per month). If one has many bulky tophi and pills do not help over time, an infusion of pegloticase (krystexxa) can be much faster but is only rarely needed.
