Most of the time you need a referral, and many insurers have "gate keeping" primary care doctors that decide f you are eligible for a a colonoscopy. Of course, if you have the cash to pay, you can usually get what you need, if not what you want.

Colonoscopy may also be done for the differential diagnosis of any form of inflammatory bowel disease not easily diagnosed from exam of stools, and there are other indications too numerous and complex to go into here.

The most important measurement that reflects the quality in colonoscopy is a doctor's adr (adenoma detection rate).
Unfortunately, there is tremendous variability in adr. The minimum should be 20. If you can't get your doctor's adr, find another doctor! doctors with adr above 20 provide ten times the protection from colorectal cancer.
