Agree with Dr. Ruiz-Healy. However, most medications are not approved for this age group (approved for 6 and older) making treatment challenging.

Hyperactive/impulsive type & disruptive behavior can occur as early as 2 1/2 if a child has abnormalities in brain structure/function from genetic or medical problems, though typical attention span can be 3-4 minutes in pre-k.
Combined-type, with h/i + inattention can occur in kg, when expected att. Span is 15 min; 20 min in 1st grade, then + 3 min/yr. It's diagnosed when symptoms impair function.

Adhd has two major parts--a) poor focus and attention and b) high activity and impulsivity.
Kids with the latter tend to be diagnosed earlier because they don't fit into social settings such as preschool. Those with both--adhd, combined type--typically are diagnosed at about age 7 (6-8) because by then peers don't have those behaviors. Those with fhe former may not be diagnosed till age 9-10.
