For a baby tooth to fall out the permanent tooth has to cause the baby tooth root to dissolve away, so it's just under the gums when the baby tooth is lost. Premature loss of baby tooth can slow eruption of permanent tooth. Crowding, impactions, non-resorbing fillings in baby teeth, tooth/jaw size discrepancies, can all affect timing and position of permanent teeth.
