In brief: 6-8000 iu, also...

I advise aiming for a level of 50-80, considered optimal by many authorities, and you probably need to take 6-8000 iu/day to reach this.
Vit k is esp. Beneficial to take, esp. Since you have osteoporosis, ; make sure you get adequate vit. A as well. See http://bit.Ly/1gnxnk0 , http://bit.Ly/1fnt3ox , http://bit.Ly/1d6mkhq , http://bit.Ly/1a48ofs ; http://bit.Ly/18qtb7y.

In brief: ? ~50,000 IU/wk

I usually wait ~3 months before rechecking d3 concentration, it takes time to change.
With osteoporosis, d3 conc. In at least the 70-80 upper normal, range probably desirable (assuming no hx ca renal stones). Also ? foods high in ca+2 (e.g. Dairy, cheese), possibly citracal for additional ca+2 may help. Depending on severity, consider recheck lunar dexa at 1 year intervals. Many otc d3 ? efficacy.

Consider researching: http://www.vitamindcouncil.org for additional issues. While this is a bit of a marketing site (despite the *.org URL) & perhaps a bit too avant-garde in the view of some (lack of mainstream medicine promoted larger double blind placebo controlled trial evidence - which is very complex & expensive to do), I believe they do offer so good information.
