In brief: Ortho eval indicated

Swelling 6 months after a bad ankle sprain may be do to several issues.
You may have sustained damage to the articular cartilage within the ankle joint, or developed some excessive scar tissue or synovitis, especially along the anterolateral aspect ("soft tissue impingement"). Nsaids and physical therapy may help, but an evaluation by your orthopaedic surgeon with possible MRI is indicated.

In brief: Many possibilities

Twisting ones’ ankle can lead to a sprained ankle.
Often times, this may result in ligament or tendon injuries which may lead to ankle bruising, pain and swelling. Swelling may last weeks to months after the initial injury. Have it re-evaluated and get the appropriate treatment.
