I am 26 male and had an inguinal hernia surgery (open repair) with mesh in 2010. Is there any possibility for scar tissue not to be shown in MRI scan?

In brief: Yes

Mri typically would not show scar tissue, unless it is extensive.
In this area, scar tissue and normal soft tissue would appear similar in an MRI scan. Consult your original surgeon for details if you are having problems with your operation.
