Should your doctor do a rectovaginal exam when you go in for your pap, just to be thorough?

In brief: Probably hot.
Unless symptoms and complaints of things like hemorrhoids or changes in bowel habits warrant it, this exam would probably not be done, and not with a pap, which is a swab of the cervix.
This would have no connection/involvement with the rectum, unless by some possible abnormality.

In brief: Yes
Ob/gyns do a complete assessment of the female genital organs, which involve visual inspection and bimanual palpation.
A recto-vaginal exam provides information that is not otherwise obvious to visual inspection, just like a bimanual assessment of the uterus and ovaries yields information that is not obvious on visual exam. A thorough exam is always better than an incomplete one.
