In brief: Doppler Ultrasound
Ultrasound has proven to be cost-effective and quite accurate in determining the degree of narrowing in the carotid artery.
Many vascular surgeons, including myself, solely rely on it to decide whether someone needs an operation. Mra is not so good and over-estimates the actual degree of narrowing. I would not rely on its findings to decide on operating. As mentioned a cta is much better.

In brief: Ultrasound
Good place to start.
If abnormal then do cta or mra. Cta better for vessel evaluation.
