If an ultrasound test result says " the gallbladder is physiologically distended " what does it mean?

In brief: Likely normal

That ultrasound reading may be a normal finding.
If symptoms of gallbladder disease persist, further testing may be helpful. A nuclear cholangiogram can sometimes show if the gallbladder is emptying properly, or if it has signs of obstruction that are not clear on the ultrasound.

Means you should have been hungry at time of test--the gallbladder normally fills (/distends) between meals, this is physiologic and not pathologic. When you eat, it contracts and discharges biliary digestive enzymes into GI tract.
