If i understand your (?) correctly, you are asking if exploratory surgery should be done.
Presently, there are very few things that we can see at laparotomy that we can't see by radiologic or endoscopic examination. Distention is either due to a solid or hollow organ in the abdomen that's enlarged--this is best evaluated non surgically; if a "surgical" problem is found, then surgery is appropriate.
